EMS Records Requests

For Patients or Family: If you would like to request medical and/or billing records, please download and complete the Medical Records Request Form and send it to Emergicon by email at support@emergicon.com or by phone at (877) 602-2060.

For Insurance Company, Record Retrieval Organization, Law Firms, etc.: If you would like to request medical and/or billing records, please submit a request through ChartSwap – register as a Records Requestor and then submit a Records Request. Or contact Emergicon directly by email at support@emergicon.com or by phone at (877) 602-2060.

If patients or requestors have any questions or require assistance, send requests to records@emergicon.com or call Emergicon’s Medical Records Department at (877)-602-2060 ext. 1611.
